Yes, it appears that Billy dies at the end of Episode 4.
Through flashback scenes, the episode relays Billy’s story. He was abused as a child by his uncle until he killed the man. He then ended up at a juvenile facility, where he developed a penchant for violence. Eventually, Billy was released from prison and became a mercenary. His work as such secured him a place in Paradise City.
Samantha “Sinatra” Redmond needs people like Billy to maintain control over the town. The episode reveals that the world above is not as inhabitable as many in Paradise City believe, and the air, in fact, is breathable. However, Sinatra, for yet unknown reasons, wants to keep that hidden. Furthermore, she is the one who sent Billy to kill scientists who discovered the truth.
However, when Xavier confronts Billy about Cal’s murder, he claims that he is not responsible, and the former believes him. It is later confirmed during Billy’s conversation with Sinatra that he didn’t indeed kill the late president. He also not only refuses to do anything to Xavier and his family and even threatens Sinatra.
This ultimately proves to be Billy’s undoing. He spends a candid moment with Xavier and his family and promises him that he will come clean about the work he has done for Sinatra the next day before returning home for the night. He and fellow agent Jane Driscoll (Nicole Brydon Bloom) have been dating for a while, and she is with him at home.
When Billy asks if she will finish her bottle of beer, she says it’s all his. He starts drinking and talking about his day before falling from the chair. As Billy dies, he learns the truth. He dared Sinatra to send someone after him. She did just that, and it was the person he perhaps least expected to betray him: Jane. Like Billy, she was one of the killers that Sinatra brought into Paradise City. Moreover, she plants pills on the table to make it look like he overdosed.
